If you’re worried about someone else
You do not need the right words. Asking directly and plainly whether someone is thinking about harming themselves does not plant the idea. It usually comes as a relief that somebody noticed.
Stay with them if you can, help them make the call rather than just passing on a number, and remove immediate means of harm where that is possible. If there is immediate danger, take them to the nearest hospital.
Once things are steadier
